Hannibal Regional Hospital, through Hannibal Regional Hospital Foundation
, has been awarded a $299,967 grant from the Missouri Foundation for Health (MFH). The funds will support MFH’s new ACCESS Grant initiative. MFH has expanded on a previous initiative, the Better Self-Management of Diabetes (BSMOD), by creating the Advancing Chronic Care through Excellence in Systems and Support (ACCESS) program. Hannibal Regional Hospital was an initial partner with MFH on the BSMOD program, which made the organization eligible to apply for this newest ACCESS initiative.
The purpose of this funding program is to increase gains made by BSMOD grantees by facilitating organizational system change, which will support sustainability and dissemination. ACCESS moves beyond diabetes as a specific condition to address chronic illness as a whole. Through this initiative, patients will have the ability to attend a health education series which will include information on the importance of self-management of chronic care issues.

and Cardiology Services
will be working with local fitness experts, including the Hannibal Y and Hannibal Regional Hospital Physical Therapy, to develop a relationship in which patients may speak with a trainer concerning specific exercise programs designed for their medical needs.
Through this initiative, telemedicine (technology which allows patients to visit with physicians over live video and audio) will be purchased and utilized to connect patients in rural healthcare clinics to the educational seminars which are held at the main hospital campus as well as to other care providers for streamlined care.
