There are four measures that are tracked for treatment of patients with Congestive Heart Failure (CHF) diagnosis. The measures include the following.
  1. Left Ventricular Failure (LVF) Assessment
  2. ACE Inhibitor Provided for Left Ventricular Systolic Dysfunction (LVSD)
  3. Discharge Instructions Provided
